Educational qualification and another criterion for empanelment of HR

Medical Officer (MBBS) ;

Total No. of Posts; 83

• MBBS from a recognized University or Institution recognized by the Medical Council of India.
• Candidates should have registered with the Punjab Medical Council/ Medical Council of India.
• Candidates should have passed the subject of Punjabi up to Matric standard.
• Age:- upto 64 years

Pharmacist

Total No. of Posts; 83

• Diploma in Pharmacy from a recognized institute.
• Should be registered as Pharmacist with the Punjab Pharmacy Council.
• Candidates should have passed the subject of Punjabi up to Matric standard.

Desirable:

• Degree in Pharmacy from a recognized institute /University
• Experience in dispensing and/or storage and dispensing of drugs in a reputed Govt. hospital or institution or in a Govt. Drug store
• Age:- upto 37 years

Clinic Assistant 

Total No. of Posts; 83

• Should have passed the Matriculation Examination / 10+2 examination with science or its equivalent from a recognized university or institution;
• Should possess a diploma in Multi Purpose Health worker (Female) / ANM from a recognized university or institution;
• Should be registered as Multipurpose Health Worker (Female) / ANM with the Punjab Nursing Registration Council
• Candidates should have passed the subject of Punjabi up to Matric standard.
• Age:- upto 37 years

Age Limit: As Mentioned above on 1st Jan 2023.

Since the AACs are IT enabled, i.e., Tablets will be issued to impaneled Medical Officers, Pharmacists, and Clinic Assistants for online Patient registration, prescription & drug dispensation, a self-declaration will be taken w.r.t. knowledge, usage & adoption of IT systems, for all the 3 categories to be impaneled, i.e, Medical Officer, Pharmacist, and Clinic Assistant. In case, a candidate refuses the self–declaration, the application will be summarily rejected.
The Candidates must ensure to bring their original & one self-attested photocopy of documents as mentioned below along with printout of the online application form at the time of the interview:-

Documents Required:

Medical Officer

1. Date of Birth certificate
2. MBBS degree
3. Foreign Medical Graduate Examination pass
4. certificate in the case of a Foreign Medical Graduate.
5. PG degree/DNB/ PG diploma certificate, as the case may be
6. Experience certificate
7. Registration certificates with Punjab Medical
8. Council/Medical Council of India.
9. Certificate in the subject of Punjabi up to matric standard or equivalent.
10. Passport size signed photograph

Pharmacist Documents Required

1. Date of Birth certificate
2. B. Pharmacy degree/ Diploma in Pharmacy
3. PG degree/ PG diploma certificate, as the case may be Experience certificate
4. Registration certificates with Punjab Pharmacy Council
5. Certificate in the subject of Punjabi up to matric standard or equivalent.
6. Passport size signed photograph

Clinic Assistant

1. Date of Birth certificate
2. Matriculation / 10+2 passing certificate
3. Multi-Purpose Health worker (Female) / ANM
4. diploma certificate
5. Experience certificate
6. Registration certificates with Punjab Nursing Council
7. Certificate in the subject of Punjabi up to matric standard or equivalent.
8. Passport size signed photograph

Important Note

• The applications can be submitted online mode only.
• Further, no notice for an interview would be issued separately to the candidates.
• No other means/mode of application will be accepted by the Department.
• The candidate can be declared ineligible due to wrong entries of his/her credentials in the form. It is advised that all entries are cross-checked by the candidates themselves before finally submitting the form.
• The office will not be responsible for the nonreceipt of the online application of the candidate due to any reason whatsoever.
• Application submitted through online form does not imply that candidate has fulfilled the criteria given in the advertisement and application is subject to subsequent scrutiny and can be rejected if the candidate does not fulfill the eligibility criteria at any point of time of the empanelment process.
